Terms and Conditions for Elysium Medical Inc.

2. Services Provided
Elysium Medical Inc. offers a range of medical services, including but not limited to:

• General consultations

• Diagnostic testing

• Preventive care

• Treatment of medical conditions

3. Appointment Policies
Patients are encouraged to schedule appointments in advance.
Cancellations should be made at least 24 hours before the scheduled appointment.
Failure to attend a scheduled appointment may possibly result in a cancellation fee.

4. Patient Responsibilities
Patients must provide accurate and complete medical history.
Patients are expected to follow treatment plans and provide feedback on their progress.
Patients must inform the clinic of any changes in their health status.

5. Financial Policies
Patients are responsible for payment for services rendered. Acceptable forms of payment include Cash, Electronic Payment Methods (GCash/Paypal), or other forms of payment methods considered eligible or acceptable.
Insurance claims may be filed on behalf of the patient when applicable, but the responsibility for payment lies with the patient.

6. Privacy Policy
We are committed to protecting your privacy and will handle your personal information in accordance with current laws on patient privacy.
Patient information will only be shared with authorized personnel in and affiliated with Elysium Medical Inc. and will not be disclosed to third parties without consent.
Patients consent to use of their anonymized medical information for data analytics, medical studies, and marketing purposes for the improvement of the medical field.

7. Consent to Treatment
Patients must provide informed consent before receiving medical treatment.
Patients have the right to ask questions and receive clear explanations about their treatment options.

8. Limitation of Liability
Elysium Medical Inc. will NOT be liable for any indirect, incidental, or consequential damages resulting from the use of our services.

9. Amendments
We reserve the right to modify these Terms and Conditions at any time. Changes will be communicated to patients, and continued use of our services constitutes acceptance of the new terms.

10. Governing Law
These Terms and Conditions shall be governed by the laws of the Republic of the Philippines.
